Thanks for the heads up. I'm not sure what the deal is, but apparently other people are having the problem as well. I've had 23 people start the survey, but only 12 people have completed it!

While it's nice to have an online version, it's also distressing to know that so many people are starting it and not completing it. However, the online version is only one option. You can still download the Word file and then send that back to me.</description>

I tried to take your survey at the link provided but couldn't complete. When I check on one question, it erases my answer in the previous questions if the frequency level is the same (???). Seems to be a code problem.</description>
