You sit for step 1 after 2nd year, because you would be taking the exam without sufficient knowledge if you took it earlier. Unless you are a foreign medical graduate, who already was practicing medicine. You can take a H&P during 1st and 2nd year, but real, significant action begins after 2nd year. Starting 3rd year.
